Toriyama forgot that the Mystic state even existed, which explains why Gohan went Super Saiyan in BoG and RoF. When reminded about it, Toriyama obviously devised a way to remedy his error to lack of remembering by having the fact Gohan had slacked in his training made him forget how to tap into that power. Gohan training with Piccolo in preparation for the ToP allowed him to remember how to tap into the power of his Mystic form.
As for Goku being surprised that Gohan didn't turn Super Saiyan, that is easily explained by the fact that Gohan had recently been using Super Saiyan instead of his Mystic form. It doesn't mean that Goku thinks that Mystic and Super Saiyan can be stacked together for a further increase in power. That would be beyond ridiculous since Mystic form allows Gohan to access the power a Super Saiyan 3 in base form. SSJ3 is a 400x multiplier and SSJ is a 50x multiplier. If they could stack together that would mean Super Saiyan Mystic form would provide a 20,000x multiplier, which is far too much.
I'm not saying you're wrong about what's been established in canon, I'm just saying it makes no sense and it seems like bad writing. I mean they probably only did that because otherwise Gohan would be unstoppable and could easily decimate most of the DBZ villains, but it seems lazy to make up stuff about his potential unlocked form that makes no sense when they could find another more sensible way to keep Gohan from curbstomping major villains, like having him busy training somewhere or on vacation with his family or something like that.
I don't see where you are getting that the Super Saiyan transformations make the body more capable of handling the power. It is just a multiplier of power and that power is part of a Saiyan's true potential seeing as Saiyans have the potential of becoming Super Saiyan. Old Kai's potential unlock just took the maximum power Gohan could achieve through Super Saiyan transformations, which was the SSJ3 form, which he hadn't unlocked and gave him all the power in his base form that that form could give him. It made it to where he had access to the power without having to transform, which itself consumes energy and without the drawback of the energy drain that SSJ3 causes. Seeing as SSJ3 is potentially possible for Gohan to achieve, then it makes perfect sense that that would be his maximum potential power. He magically made it to where Gohan could tap into all of that power whenever he wanted without needing to transform. As for making sense, magic always doesn't make sense in Dragon Ball, but to be honest it makes sense in this situation.
