Well this question is tricky, as usually adding a new character means a new universe and that is a different question altogether. Though I suppose if I had to pick one, Dr. Gero.
The fact that the man who created the androids and Cell, the guy with ties to the Red Ribbon Army arguably one of the most significant villain factions in DB has zero representation in DBM even as a spectator is honestly to crime.

For me I would say Hit, given the divide between magic and traditional fighters (like vegetto), his timeskip would had served as a good in-between the two types of powers.
Even if the damage Hit deals was toned down, the time skip itself offers interesting possibilities.

The Difference being its after the buu saga and since buu was so evil it clogged up the soul purifier(or what ever its called) (and it's caused by Goku since their trying to Reincarnate Kid Buu's Soul.
so hell opens and souls from hell invade earth, with Gohan staying on earth with the rest of the Z-fighters and Goku and Vegeta going to hell to deal with the source.
Then Rest of Fusion Reborn Plays out.

The Shadow Dragons or at least Omega Shenron. I always thought they were a cool concept that could have worked a lot better if the execution was done differently from what DBGT did with them. There were a few cool concepts to GT that if it was executed better could have been great in DB canon. The Dragon Balls being overused causes this negative consequence was a neat idea to me. While most of the shadow dragons turned out to be meh threats if reimagined I could see a scenario where they could have made a great group of villains that challenge our heroes. DBM wouldn’t need to do a lot of work to create a universe where they or Omega Shenron won and terrorized their universe. Their motive for entering the tournament could easily be they wish to spread out to other universes, possibly corrupt other Dragon Balls in those universes to create an army to conquer or terrorize the multiverse. If the story wants to be different it could frame them as liberators who are trying to free their counterparts from the other Dragon Balls. A lot could be done with the concept of the Shadow Dragons.
